A great variety of medical conditions are subject to the placebo effect. Although there is mounting evidence to suggest that the placebo effect is related to the expectation of clinical benefit, little is still known about the biochemical bases underlying placebo responses. Positron emission tomography studies have recently shown that the placebo effect in Parkinson’s disease, pain, and depression is related to the activation of the limbic circuitry. The observation that placebo administration induces the release of dopamine in the ventral (...) striatum of patients with Parkinson’s disease suggests a link between the placebo effect and reward mechanisms. In addition to Parkinson’s disease, the placebo-reward model may also apply to other disorders. However, the relative contribution of the different neurotransmitters and neuropeptides that are known to be involved in modulating the activity of the limbic system may be disease-specific. Thus, while the placebo-induced clinical benefit observed in Parkinson’s disease would mostly reflect the release of dopamine in the dorsal striatum, the activation of opioid and serotonin pathways could be particularly implicated in mediating placebo responses encountered in pain and depression, respectively. This paper deals with a major problem of the early days of the science of palaeontology, the nature and origin of fossils. In 1725, the German physician and academician, Dr Johann Beringer, was deceived into believing that fraudulent, carved stones were true fossils. Despite an extensive analysis, Beringer was not able to fit his ‘fossils’ to any contemporary concept of how fossils originate. The reason for Beringer's inability to classify his stones is explored here. It is argued that the carvings (...) embodied a sixteenth-century, Neoplatonic, concept of fossils, a concept that was outdated in Beringer's time. Beringer chose not to explain his stones rather than to accept the meaning they implied. (shrink)

Davis is a Swedish American mixed-media artist who was born in Lund, Sweden, and currently resides in Washington, DC. Her work draws frequently on the challenges she has faced living in the United States, particularly those related to her gender and racial position. Her husband, who spent (...) most of his childhood in the same Swedish town as Davis, was born to an African American man and a white Swedish woman. When Anna, who is also white, was pregnant with her own son, now in his early twenties, she began drawing what she terms “Frocasians,” people who are neither black nor white and who cannot claim automatic acceptance from either group. As her work explores, the outsider status of Frocasions both presents opportunity and exacerbates insecurity. Davis’s Swedish roots are central to her brand of feminism. She remembers growing up in an environment where notions of gender justice were firmly embedded in the legal framework and norms of the community. Her struggles with inadequate childcare and family leave as a young mother in the United States, then, form a strong undercurrent shaping her critical perspective. Davis also narrates growing up in a household that was emotionally unstable and sometimes difficult to navigate. We reflect on Dr. Iserson’s article in this journal, in which he suggests that clinicians must “shove” patients towards appropriate care. While recognizing that overt clinical guidance is part of care, we suggest that its use should be tempered by the guidance’s responsiveness to the human and emotional experience of each patient.

Dr. McGee presents a cogent argument for the patentability of the diagnosis of gene forms that are found to be associated with disease or other phenotypic manifestations. We're convinced he's wrong. An analogy will help explain why.

The case of Dr. Nancy Olivieri, the Hospital for Sick Children, the University of Toronto, and Apotex Inc. vividly illustrates many of the issues central to contemporary health research and the safety of research participants. First, it exemplifies the financial and health stakes in such research. Second, it shows deficits in the ways in which research is governed. Finally, it was and remains relevant not only in Toronto but in communities across Canada and well beyond its borders because, absent appropriate (...) policies, what happened in Toronto could have happened (and could well still happen) elsewhere. In Part One of this paper, we review the facts of the Olivieri case relevant to the issues we wish to highlight: first, the right of participants in a clinical trial to be informed of a risk that an investigator had identified during the course of the trial and the obligation of the investigator to inform participants (both her own and those of other investigators); and second, the obligation of institutions to protect and promote the well-being of research participants as well as academic freedom and research integrity, the obligations of research sponsors to inform participants, research regulators, and others about unforeseen risks, and the obligations of research regulators to ensure that participants are informed of unforeseen risks and to otherwise protect and promote research integrity. In Part Two, we relate these facts and issues to New Zealand and Australia. We also make detailed recommendations for changes to the various instruments used for the governance of research involving humans in Australasia. (shrink)

The essays in this volume consider the question of whether the self is a unity or whether it should be conceived without metaphor as divided - as a 'multiple self'. The issue is a central one for several disciplines. It bears directly on the account of rationality and the explanation of individual decision-making and behaviour. Is the hypothesis of a multiple self required to deal with the problems of self-deception and weakness of will; and can the conceptual tools developed in (...) the study of interpersonal conflict be applied to the analysis of intra-personal struggle? Most of the essays, by a number of leading philosophers, psychologists and economists, appear here for the first time. They bring out the interdisciplinary importance of the question, and will interest readers in all those areas. Der Krankheitsbegriff soll aus der Perspektive der klinischen Medizin am Beispiel der Radioonkologie dargestellt werden. Als theoretisches Modell zum ganzheitlichen Verständnis wird zunächst der interaktive Dualismus eingeführt. Danach wird die Interaktion zwischen Geist und Körper als wesentlich angesehen, um den Patienten mit Würde und Mitgefühl zu behandeln. Auch die Strahlentherapie erfordert nach ihrem Selbstverständnis einen ganzheitlichen Zugang zum Patienten. Demgegenüber zeigt die Analyse der aktuellen radioonkologischen Literatur einen reduktionistischen Krankheitsbegriff, der in erster Linie objektive Kriterien aus ärztlicher Sicht zur Definition (...) von Krankheit heranzieht. Im Dialog könnten stattdessen Arzt und Patient einen gemeinsamen Krankheitsbegriff entwickeln; hierbei finden im Sinne des interaktiven Dualismus gleichermaßen objektive ärztliche wie subjektive patientenbezogene Kriterien Berücksichtigung. The occasion of this talk was a panel discussion ending the conference on “Radical Politics” sponsored by The University of North Dakota’s chapter to Students for a Democratic Society. Many of the members are self-described Marxists and Anarchists. It is they whom I address here.

Numerous accounting and economics research studies employ an experimental research method requiring student participants to make representations about an individual characteristic (e.g., ability, cost) that provides a basis for payment of cash rewards. In response, many participants intentionally misrepresent the nature of that characteristic to receive a greater reward. Typically, such studies are deemed to be either exempt from review by institutional review boards (IRBs) or subject only to an expedited review. Moreover, investigators seldom debrief participants, purportedly to avoid contamination (...) of the participant pool. The authors question the ethics of inducing strategic misrepresentation and rewarding fraudulent behavior without appropriate debriefing. A theoretical framework of cognitive ethical development indicates that such research methods can be harmful to student participants. Furthermore, principles of operant conditioning suggest that the rewards reinforce fraudulent behavior. It is recommended that research studies inducing strategic misrepresentation should not be exempt from IRB review and should be required to include desensitization procedures for debriefing participants. Michel Foucault's involvement with politics, both as an individual and a writer, has been much commented upon but until now has not been systematically reviewed. This is the first major introductory study of Michel Foucault as a political thinker. Jonathon Simons explores the importance of the political in all areas of Foucault's work and life, including important material only recently made available and the implications of various revelations about his private life. Simons relates Foucault's work both to contemporary political thinkers (...) such as Michael Walzer, Charles Taylor and Jurgen Habermas, and to those challenging conventional political categories, especially people who write on feminist and gay theory, such as Judith Butler. First of all, I would like to commend Roberts and colleagues for taking on a difficult but very important topic. It would be valuable if someone could follow up with a broader sample of universities and laboratories — paying careful attention to possible sampling and non-sampling errors. In general, I recommend that mentors explicitly both learn and teach ethical theory and practice within the context of their scientist development programs. Finally, while it is important to emphasize sound and ethical research (...) standards and processes in mentoring programs, this should not be at the expense of an appropriate emphasis on skills essential to professional competitiveness, such as responsible, effective publication, grant winning, and demonstrably strong performance on the grants won. How an abstract immaterial being is connected to a physical thing has been viewed variously by western philosophers who considered the issue prior to their Muslim counterparts.Muslim theologians and philosophers, however, developed the related discussions which became heated following the translation of logical books and essays throughout the Translation Era.The focus of this article, besides clarifying the ideas raised by Muslim philosophers in this regard,is to shed light on Mulla Sadra's opinion and its influence on the later philosophers, Abd-ul-Razzaq Lahiji (...) in particular.To explain his opinion,Mulla Sadra primarily brings a proof to establish the unity of the soul,and to prove this,he has refuted the reasons used by the opposing philosophers through negation and affirmation.Using negation,he comments on why there can't be a unity of soul, and then refers to his opponent's inability to distinguish between specific unity and generic unity considering the latter equal to vegetable,animal and human spirits.Soul and body,Mulla Sadra reiterates,are attached to each other in two ways; in one way,soul is attached to body and yet it is contingent on itself and not on the person's being; in the other way,soul is attached to body while the person's potentialities are actualized; this second type of attachment relates to obtaining existential virtues.It must be noted that Mulla Sadra's belief in these two types of attachment results from his belief in the trans-substantial motion in which soul is also involved.Mulla Sadra works out a solution to the question of relation between soul and body and their effects on one another by proposing the mediation of a veiled spirit, since the soul which belongs to a world of pure light does not involve itself directly in the affairs of the dark body. The veiled spirit is as subtle as the non-material soul and as corporeal as it needs in connectionwith body.This view has been argued by Allameh Tabatabii who thinks corporeal qualities and ontological status must be kept separated and not be mixed.Lahiji,on the other hand, seems to have accepted Mulla Sadra's idea looking at it as a philosophical question,not a medical case. He,like Mulla Sadra, while considering unity for soul, thinks of it as having properties such as sensation imagination estimation intellection. According to Lahiji,the unity of soul and body is like the unity of matter and form; however,unlike natural forms,it does not subsist in matter and interestingly,this notion resembles Neo-Platonist views. Despite the skepticism of many mathematicians and logicians as to the possibility of any test which will show conclusively the consistency or independence of the members of a postulate set, several methods have nevertheless been devised and employed, e.g., the empirical methods of Russell and Huntington, the internal method of Hilbert, and the reflective method of Royce. However, with the possible exception of Hilbert's method, these techniques require us to forsake the purely formal or abstract mode of analysis, and instead (...) to rely in some sense on “concrete representations” or “interpretations” or “modes of action” which are said to “satisfy” the postulates in question. In short, they are all interpretational methods—and here is the crux of the problem. “Is it possible,” queries Weiss, “that the only way we can determine whether a set is consistent is by seeing all the postulates actually exemplified in some object? If so, we must arbitrarily assume that the object is self-consistent, so that the proof of consistency must ultimately rest on a dogma. As independence rests on consistency there are therefore no satisfactory proofs as yet of either independence or consistency.” The question “is thus answered,” according to Young, “only by reference to a concrete representation of the abstract ideas involved, and it is such concrete representations that we wished especially to avoid. At the present time, however, no absolute test for consistency is known.” Moreover, “suppose,” says the late Dr. Eaton, “that the system has no interpretation: how can the consistency independence … of the postulates be shown? There should be some analytic way—purely in the realm of the abstract, without interpretation—of establishing these properties of a set of postulates. The present article aims to explain Kamala??la’s understanding of the nature of insight, specifically considering it as the ‘discernment of reality’ -- a technical term identified with insight in the author’s well known Bh?van?krama? texts. I approach my analysis of bh?ta-pratyavek?? from three different angles. I begin by providing a rationale for its translation. This is followed by an account of Kamala??la’s reading of key passages in the La?k?vat?ra S?tra describing the process to which the term refers. Here the aim (...) is to illustrate Kamala??la’s understanding of bh?ta-pratyavek?? as it is actually experienced in meditation. The final section examines bh?ta-pratyavek?? in relation to other important technical terminology employed in the course of making arguments against his historical rival in debate, the Ch’an monk Mo ho yen. By providing these three different perspectives on the same process it is my hope that both scholars and practitioners will be able to more fully comprehend and benefit from the instructions provided by the ancient master Kamala??la. Feminist philosophers of religion have drawn attention to desire as a neglected category for approaching the sources and concerns of religion. This paper extends this discussion by engaging with one particularly disturbing aspect of the writings of the Marquis de Sade. In a world where ultimate sexual pleasure is derived from destruction of the Other, Sade glories in describing the suffering of mothers, often at the hands of their own children. This paper offers one possible reading of these dark desires (...) through employing aspects of psychoanalytic theory to suggest that, rather than isolate him from the majority of humanity, we might consider possible connections between the desires he details and our own. This involves considering the ambivalence of the mother, and thus challenging the idealization of the all-caring, all-nurturing mother. (shrink)

The city of Samarra is one of the major cities of Iraq that Yaqout al-Hamwi described in the dictionary of countries by saying (it became the greatest country of God) and Al-Qazwini told about it in the monuments of the country (a great city located on the outskirts of the east of the Tigris between Baghdad and Tikrit) built by the Abbasid Caliph Al -Mu'tasim in God in 221 AH/ 835 M, and it is originally a monastery for the Christians, (...) who bought it for 5,000 dinars, and took it as a capital for its succession, and reached its prosperity and its glory is the days of the caliph Al -Mutawakkil, 247 AH / 861 AD / 892. However, after 58 years, it was left as the capital of the Abbasids when the Abbasid Caliph, Al-Mutamid Allah, abandoned it in the year 279 AH / 892 AD, and took Baghdad, the old capital, as his headquarters, which maintained its position as the capital of the Arab Islamic state until the fall of the Abbasid Caliphate at the hands of the Mongols in the year 656 AH / 1258 AD.Al-Mu'tasim and the caliphs and princes who came after him built palaces, houses, mosques, and other things in Samarra. Samarra was famous for its Great Mosque, distinguished by its high, twisted minaret. It also contained the shrine of the two imams, Ali al-Hadi 254 AH/868 AD and Hasan al-Askari 260 AH/873 AD.
